MISSION AND OBJECTIVES

Intensive Community Response Team

CONTEXT OF THE PROJECT

Too often, a period of homelessness associated with substance abuse can have a devastating impact on a young person's future: extensive drug use, the stress associated with the homelessness and the trauma that can be experienced increase vulnerability to developing serious mental health problems. Conversely, the disorganization associated with severe psychiatric problems combined with substance abuse too often leads to homelessness.

These young people, who are difficult to reach, often do not receive the necessary care in time and experience the worst consequences of these physical and mental health problems. The main objective of this project is to give these young people back their future. The project aims to provide those suffering from severe mental health disorders and addiction with a path out of homelessness by combining medical, social, housing and a return to productive life within the same outreach team. This outreach team will lend a hand to the teams at the Clinic for Young Adults with Psychotherapy (JAP) and the Addiction Psychiatry Service (SPT) at the CHUM, in order to be able to offer outreach services more intensively to this clientele.

À qui adresse le projet?

  • Jeunes adultes âgés entre 18-30 ans
  • À risque d’itinérance ou vivant dans la rue
  • Présentant un problème de toxicomanie et un trouble de santé mental sévères émergents et n’ayant pas eu accès au traitement approprié pour les troubles concomitants.

Le premier contact  peut avoir lieu, soit:

  • Dans un organisme fréquenté par le jeune ou dans nos bureaux situé au Centre hospitalier de l’Université de Montréal.
  • Le jeune peut venir seul ou accompagné des intervenants avec qui il a développé un lien de confiance.

  • Promote the early detection of concurrent substance use and severe mental health issues among youth experiencing homelessness.
  • Reduce the time lag between the onset of mental health/substance abuse problems and access to appropriate treatment.
  • Offer treatment in the young person's living environment based on collaboration with partner organizations.
  • Decrease the physical, psychological and social complications associated with psychiatric-substance abuse and homelessness.

OBJECTIVES

  • To improve the accessibility of addiction treatment, psychiatry, rehabilitation and social reintegration services for street-involved youth with substance abuse and severe mental health disorders.
  • Improve the continuity of services offered to street youth in collaboration with the community organizations that work with them.
  • Stabilize the situation of individuals in terms of mental and physical health, housing and consumption.

AWARDS AND DISTINCTIONS

The ÉQIIP d'intervention intensive de proximité - SOL was awarded an honourable mention at the 2014 Health and Social Services Awards of Excellence (category: Accessibility to care and services). The ÉQIIP d'intervention intensive de proximité - SOL was also a finalist for the 2020 Health and Social Services Awards of Excellence (category: Partnership) for the Réseau d'intervention de proximité auprès des jeunes de la rue.

SERVICES OFFERED AND PARTNERS

EVALUATION

Initial contact will be made within 24 hours (business days) of the request to determine if the person is eligible for the services offered by the project, or to help refer them to the appropriate services.

Further assessment by a psychiatrist and psychosocial intervention professionals may take place if necessary, to determine the person's needs and refer them to appropriate treatment.

TREATMENT

The project aims to bridge the gap between services offered in the community and medical and psychosocial treatments specific to the targeted clientele. Treatment prioritizes interventions in the living environment. Joint follow-ups with the workers already involved with the young person are therefore a priority.

Interventions focus on developing independence in vocational (employment/education), social (developing social networks, integrating community resources that meet their needs), and psychological (better understanding of their health problems and the impact of substance abuse on them) areas.

Following the assessment, there will be suggestions and referrals to treatment from the JAP Clinic, PTS or other resources that will be offered to the youth in order to stabilize psychiatric symptoms and/or substance abuse and homelessness disorders.
