if you have early psychosis and live in the CSSS Jeanne-Mance and Cœur-de-l'Île catchment area.
Your postal code must begin with one of the following three-letter prefixes:
H2H, H2K, H2L, H2T, H2V, H2W,

H2X, H2Y, H2Z, H3B, H5B or H2J.

The first contact can take place:

• At community organizations the young person already attends
• At our offices at the CHUM

You may come alone or with clinicians with whom you have built a trusting relationship.
